I have a child in my care that has been having problems having a poo - she is now on medication which we originally thought was only going to be a couple of weeks but she has arrived this morning (been on holiday last 10 days) and mum has told me that they have been back to the doctors and things still haven't massively improved so she will be on the medication for quite some time.
I have a standard permission form for prescribed medications but usually as they are only given short term, I get the parent to bring completed form every day with doses and times to be given etc. As this is going to be a long term thing I think it very silly that a new form has to be filled in everyday and was going to type new one up that would cover a longer period - probably a month at a time.
I haven't had a child in a long time that needed long term medication (have long time ago before we had to be as strict with paperwork!!) and was wondering what was the best way to go about this? Has anyone got a child on long term medication?
I can type up a standard form easily enough and get mum to sign them each month but what do you do to log that the medication was actually given. We never used to worry about this in the past but bearing in mind how fussy Ofsted are now I am trying to figure out the best way of proving/logging that the medication was given, dosage, time,date etc and do you get the parent to sign as acknowledgement, if so how often do you get them to sign it i.e. do you do list all relevent details daily and just get them to sign for a week at a time?
I hope I am making sense but I would be really grateful if you would be willing to share ideas with me and point me in the right direction because at the moment I don't really know what to do for the best.
