What shall I put on a telephone enquiry sheet?

I am just trying to make myself a reminder sheet for questions to ask whena potential client phones.

So far I have:

Parent name

Contact number

Childs name

Days/hours required

What else should I ask on an initial inquiry?

any special requirements for the child?

childs age

I need to type mine up, didnt get round to it yesterday

Make sure they know what area you live
The amount of people that call me are the other side of city so best to ask before you write all their details down

if they will need to be taken to nursery or playgroup. It's another thing some parents assume - that you will take them to whatever nursery they want, no matter that it's too far or you have other commitments.
