I do my own which are for:
Routine Outings - including walking to and from school Transportation by car or public transport Dental Hygiene - if they want you to help with teeth cleaning after meals Observations - for any course works on development books Medication Urgent Medical Treatment Sun Cream application
and Photographs but I use the NCMA form for that as it is more detailed and covers websites etc.

hi Pauline i have all of them except the dental hygiene one is that if you are helping cleaning the child's teeth? i do have a child that has to clean her teeth in the morning after breakfast but she is 6 would i she need a consent form for that age?

Yes that's the one.
You don't have to have it but it all helps keep Ofsted happy.
Mine reads:
I/we agree for the above named childminder to help the above named child with dental hygiene and teeth cleaning. I/We will supply a suitable toothbrush and toothpaste and replace the toothbrush and paste on a regular basis.
It just helps to cover you against any claims that a child became ill after using toothpaste they were allergic to etc. or something like that.
All these permission forms are to protect yourself from claims for any detrimental outcome of doing things with the children.
But as I say you don't have to have some of the lesser ones, it just shows good practice if you do.
The most important are all the others I listed, everyone should have those.

Also
Taking photos which is a different one to the observations
Permission for pics on my website and my newsletter
Permission to play on outdoor climbing frames ie in my garden and the park
Sand pit
Also a trampoline or bouncy castle ( If we go out anywhere for the day ) there seem to be so many dangers on these so I did a permission form for this too to cover myself

hi i don't have the teeth cleaning one but i do have one about allowing toddlers to sleep in their push chair and notifying parents that i have a cat, they have to tick the box to show that they are aware and that it isn't a problem for him to be around their kids. my network coordinator told me that some minders were told by ofsted that they needed one i also have an outings one stating that i will notify parents 24 hours in advance of any trips or outings i have a photos one for my portfolio and website but i also have a photos one for the press as some times the press come to organised events and i wouldn't want my mindees to miss out being in the paper
