I have not got a allergy chart
When I worked in a nursery a few years ago they had one - but this is because there was over 100 children we would never have remembered who could have what for lunch etc without it
I would not have thought you needed one for minding
Our allergy chart at nursery was based on the points that Donna made
Is this the same as what you meant ?

I have an allergy chart to be completed for each child. Not that I have any mindees yet.
It is a form to be completed by parents stating if child has an allergy, what reaction they have to it, how it is treated etc. I plan to keep them inside a cupboard out of reach of children in the kitchen to refer to.
Doesn't sound like it is a compulsory form though. Wiggly x

Yes I have a short form for parents if I need it for allergy but it is not a chart

I just have list of initials and the foods they aren't allowed to have in the front of my diary. Got no allergic mindees at the moment but a number of requests from various families. Much easier to refer to this than to dig out the files each time. I've got an apalling memory and I can't easily remember which parent has given which instruction.
I gave a child kiwi once - she wasn't allergic but mum had requested she's not to have it. Mum was brilliant about it but I'm shell-shocked to have made such a basic mistake and can't help thinking what might have happened if she'd had a reaction. I'm not going to make that mistake twice.

its not compulsory just makes life a little easier when i plan to put a list up in my kitchen as i have several children who arnt allowed certain foods or are unable to have them i just wondered if anyone else used them or was i going overboard

I do have A5 forms which can have a photo of the child on it, name of child, allergy, reaction if given, how to treat it, foods allowed/not allowed, drinks allowed (ie some parents say water/milk only - no squash) etc etc etc.
I don't plan to mind many children so I will put any allergy sheet (if any children have an allergy) on the inside of my kitchen cupboards (where I keep plates/cups and will see on daily basis). I was advised to keep them in cupboard and not on display for confidentiality reasons.
