ok, i've not yet received my ncma pack yet and i have a parent coming tomorrow night, so i'm having to devise my own stuff.
Ok the permission forms i have are photos, sun block, travelling cars / buses etc, routine outings, out door play equipment - do i a permission form for anything else?

hi peggy, first of all thankyou just getting started and completely forgot about photo permission, the only other ine i can think of for you is permission for emergency medical treatment just in case theres a mad dash to hospital. thank for reminding me anne-marie x

These are all the forms I give to parents before they start:-
Application Form - so parent commits in writing which days and hours they want so you don't get the situation I got with my first customer who insisted she'd never asked for a Wednesday (she did, I wrote it in different places and turned away a friend who asked for the same day).
Page 1 - Details of Child Page 2 - Parent/Guardian contact details, place of work etc
Permission Forms A - Who can collect the child B - Day to Day Trips C - Emergency Treatment D - Photo Permission E - Observation Permission F - Animals On Premises G - Face Painting H - Sweets & Treats I - Administering Medicines (includes sun cream, plasters, teething gel etc) J - (older children only ) Consent to walk from school unaccompanied
Accident form (duplicate emergency permission is on top of this form - I was told I need this separate too, as it may end up in the hands of ambulance staff/hospital) Child Protection Statement

I know this reply is too late for peggy but i had my pre-reg inspection yesterday and the lady suggested that I put all of the things that need parental permission signatures on to one sheet of paper meaning that the parents/guardians only have to read and sign once and also amending the list and asking the parents to sign again for additional things would not be a big issue. It would also mean less paperwork in each child's individual folder. This might be something that you all do as standard but it seemed like a good idea to me and was something that I hadn't thought of so it might help somebody
