Does anyone know how to do a risk assessment form, I have a boy starting with debetes and i dont have a clue. I really need your help.

I would think about getting together with his mum and writing a care plan for him this should include - what sort of diabetes he suffers from, how it is controlled, whether you need specialist training, how to protect him from suffering an attack, what to do if he does get ill, what symptoms you should be looking out for, what food / drinks he should be having and what to do if something slips past you web links to recipes, people you will work alongside in his care eg chiropodist, dietician, doctor, consultant permission to seek their expert advice if mum or dad aren't contactable exercise guidelines for diabetic children whether he is registered disabled if you are expected to / willing to take him to appointments for his condition what support he will need if you take /collect from nursery / school etc Hope this helps! The advice Sarah gave you was really good
If you follow her guide lines then do your care plan based on those
Just do a list if you are unsure
Eg
Q ..What sort of diabetes does ...... suffer from ? A....Then explain what he suffers from and how long
Q....Is the diabetes controlled ? A...eg .. yes it is controlled by .......... or no it is not controlled
Q....Do you need training? A...You could explain that you have asked parent and this is what she said ................................... So you are learning day by day
Q...What symptoms should you look for? A....Give a detailed explaination about this based on what parent tells you
Q....What food / drink he can have or what can he not have A....Explain IN detail
That sort of thing

Hi; the others have given you some good advice
You need always work closely with parent and keep each other informed.
My eldest son was diagnosed with diabeties age 3 i used to get very concerned when he was not in my care and would always make sure anyone looking after him was aware and knew what to do so i'm sure parent will keep you well imformed.
The others have covered what you need to know
