Quick question do you have to provide parents with a copy of a completed accident/incident form or just get the parents to sign them Strawbsxx

I always ask - do you want a copy? - they usually say 'no' And I remind them the original is in their child's file Sarahx

I always give parent a copy as i feel i am covering myself then they cant say further down the line that they were not told about the accident Lindaxx

I use my own form and write out details of the accident and then photocopy. Get the parent to sign my copy and send home the other for their records.

I always give them the copy, I use NCMA forms. That way they have had the copy and I've covered myself from any comeback.

I get the parents to sign the original (print off my own from the computer) then photocopy it with both signatures on. I also number each sheet and then these are logged on an accident log sheet with the date, time, childs initials so I can cross reference to the childs folder also provides confidentiality as can prove I have them to any assessor but they don't see original if that makes sense.
I have a permission slip that a parent signs if they do not want a copy 2 parents do one doesn't as she only puts it straight into the bin.
